Heath Services for out of hospital care in Fremantle

Patients in the Fremantle area will have access to out of hospital care in state-of-the-art facilities due to the recent expansion of ambulatory care services at Fremantle Hospital and Health Service. Health Minister Dr Kim Hames today opened the refurbished facilities, which include two physiotherapy gymnasiums, a hand therapy unit and Hospital in the Home facilities. “Ambulatory care provides services in home and clinical settings - helping to reduce pressure on hospital beds,” Dr Hames said. “Many acute and chronic medical conditions can be efficiently managed by a combination of hospital-based or clinic-based services and home visits, which is what these services provide. “These new ambulatory care facilities and services will help patients return home while reducing hospital re-admissions and preventing inappropriate entry into aged care homes.” The Fremantle Hospital in the Home service provides daily hospital-based care to about 60 patients in their own homes. The Minister said the service allowed patients to be transferred to a ‘virtual’ ward where they could be nursed by a team from their local area. “The new facilities will provide expert care to patients recovering from a range of illnesses, and offer individual consultation rooms and room to exercise, along with new rehabilitation and exercise equipment,” he said. “They also complement other services offered to patients in the Fremantle area such as the hydrotherapy pool.”

The purpose-built facilities are a joint State-Commonwealth initiative totalling $1.35million. The two physiotherapy gymnasiums ($690,000), Hospital in the Home ($425,000) and Hand Therapy Clinic ($240,000) were the beneficiaries of Commonwealth Government Pathways Home funding.
